Disconnected pancreatic duct syndrome (DPDS) is a circumferential disruption of the continuity of the pancreatic duct resulting from a complication of necrotizing acute pancreatitis. On some occasions, this entity represents a diagnostic dilemma because the viable pancreatic tissue is not visualized with the image methods, which finish in a recurrence of the collection after the initial drainage. Currently, EUS-guided drainage is the gold standard in the treatment, with the long-term placement of plastic stents. We present the case of a 39 y.o. male patient with previous percutaneous drainage and posterior recurrence of the pancreatic collection. The drainage was performed with a unique double pigtail stent with an excellent evolution 8 days later.
